Healthcare Boost For Over 60's
A new project to improve healthcare for the over 60s in Northamptonshire aims at providing a more consistent approach to their care.
A new project to improve healthcare for the over 60s in Northamptonshire should be able to provide a more consistent approach to their care.
The scheme is called the Elderly and Frail project and it has seen social care workers and health care workers work together from one office to coordinate care. The aim is to reduce the number of over sixties admitted to hospital and to reduce the length of stay for those who are in hospital.
Two members of the team, Elaine Crisp and Alison Sheppard, explain how it works and Linda, daughter of a lady who was recently discharged from Northampton General Hospital, describes how things have changed.
Stuart talks to Keon Tysoe, a community carer who describes what happened to an elderly man he cares for. Stuart also talks to David Oliver, a Kings Fund Fellow who is also a geriatrician who has written several papers on improving care of the elderly.
